Guía para el manejo multimodal del dolor postoperatorio en adultos mayores

Abstract

El manejo del dolor postoperatorio representa un desafío particular en la población geriátrica, principalmente a los cambios fisiológicos asociados al envejecimiento y a la alta prevalencia de comorbilidades. El manejo inadecuado del dolor en pacientes quirúrgicos en edad avanzada puede aumentar la morbilidad postoperatoria, contribuyendo a la aparición de delirium, complicaciones cardiorrespiratorias y dificultades en la movilización. Esta guía busca destacar la importancia de comprender la fisiopatología del dolor postoperatorio agudo y crónico, así como la necesidad de reconocer adecuadamente el dolor en los adultos mayores, con el fin de optimizar el manejo del dolor a través de un manejo multimodal y así poder reducir las complicaciones postoperatorias y prevenir la transición al dolor crónico. The management of postoperative pain represents a particular challenge in the elderly population, mainly due to the physiological changes associated with aging and the high prevalence of comorbidities. Inadequate pain management in elderly surgical patients can increase postoperative morbidity, contributing to the onset of delirium, cardiorespiratory complications, and mobility difficulties. This guide seeks to highlight the importance of understanding the pathophysiology of acute and chronic postoperative pain, as well as the need to adequately recognize pain in older adults, in order to optimize pain management through multimodal management and thus reduce postoperative complications and prevent the transition to chronic pain.
