Caracterización y descripción de la evolución clínica de los pacientes diagnosticados con leucemia linfocítica crónica en el Hospital San Juan de Dios de enero 2023 a diciembre 2024

dc.description.abstractIntroducción: la leucemia linfocítica crónica (LLC) es la leucemia más frecuente en adultos. Su manejo se ha transformado con la incorporación de marcadores moleculares y terapias dirigidas. Asimismo, en Costa Rica, el único antecedente institucional disponible data del año 2016, dicha situación caracterizó a esta población en el contexto diagnóstico y terapéutico actual. Objetivo: describir las características clínicas, biológicas y terapéuticas, así como los desenlaces, de los pacientes diagnosticados con LLC en el Hospital San Juan de Dios (HSJD) entre enero 2023 y diciembre 2024. Metodología: estudio observacional, descriptivo-analítico, de cohorte retrospectiva y carácter censal. En el cual, se incluyeron 34 pacientes diagnosticados por citometría de flujo según criterios iwCLL 2018. La fecha de corte fue el 2 de mayo de 2026. Se realizaron análisis de supervivencia por Kaplan-Meier y comparaciones exploratorias mediante log-rank. Resultados: la mediana de edad fue 69 años, con predominio masculino (razón 1.3:1). La mayoría se diagnosticó en estadios tempranos (Rai 0-II: 73.5 %; Binet A: 61.8 %). Por su parte, la caracterización molecular fue limitada (FISH en 18-26 %; IGHV y TP53 en 17.6 %), por lo tanto, impidió la estratificación pronóstica completa. De igual forma, la estrategia predominante fue la observación activa (70.6 %); el 29.4 % recibió tratamiento, mayoritariamente con quimioinmunoterapia, accediendo a terapias dirigidas hasta segunda o tercera línea. La mediana de supervivencia global no se alcanzó (seguimiento mediano 31 meses). El alto riesgo según Rai (p=0.022) y la anemia al diagnóstico (p=0.044) se asociaron con peor supervivencia. Conclusión: considerada la primera descripción institucional que incorpora FISH y terapias dirigidas. Además, persisten brechas en la caracterización molecular y en el acceso temprano a terapias dirigidas, cuya corrección permitiría la optimización del manejo de estos pacientes.
dc.description.abstractIntroduction: Chronic lymphocytic leukemia (CLL) is the most common leukemia in adults. Its management has been transformed by the incorporation of molecular markers and targeted therapies. In Costa Rica, the only available institutional reference dates from 2016, which motivated the characterization of this patient population within the current diagnostic and therapeutic context. Objective: To describe the clinical, biological, and therapeutic characteristics, as well as the outcomes, of patients diagnosed with CLL at Hospital San Juan de Dios (HSJD) between January 2023 and December 2024. Methods: Observational, descriptive-analytical, retrospective cohort study of a census nature. A total of 34 patients diagnosed by flow cytometry according to the 2018 iwCLL criteria were included. The cutoff date was May 2, 2026. Survival was analyzed using the Kaplan-Meier method, with exploratory log-rank comparisons. Results: Median age was 69 years, with male predominance (ratio 1.3:1). Most patients were diagnosed at early stages (Rai 0-II: 73.5%; Binet A: 61.8%). Molecular characterization was limited (FISH in 18-26%; IGHV and TP53 in 17.6%), precluding complete prognostic stratification. Active surveillance was the predominant strategy (70.6%); 29.4% received treatment, mostly chemoimmunotherapy, with access to targeted therapies only at second or third line. Median overall survival was not reached (median follow-up 31 months). High-risk Rai stage (p=0.022) and anemia at diagnosis (p=0.044) were associated with worse survival. Conclusion: This is the first institutional description incorporating FISH and targeted therapies. Gaps persist in molecular characterization and in early access to targeted therapies; addressing these would optimize the management of these patients.
